The NC Administrative Office of the Courts publishes and requires statewide small-claims forms used directly (not mere illustrations to be modified): AOC-CVM-100 (Magistrate Summons), AOC-CVM-200 (Complaint For Money Owed), AOC-CVM-201 (Complaint In Summary Ejectment), AOC-CVM-303 (Notice of Appeal To District Court), plus claim-specific forms (personal property recovery, motor vehicle liens). AOC-CVM-200 itself requires attaching a copy of the account, promissory note, or check when the claim is based on one of those categories. Local clerks may require additional copies; confirm exact quantities with the county clerk.
